Individualized community access support and residential programs for persons under 18. Services are provided in employment, vocational, living skills development and community access.

 

 

 
  Functional assessment of parenting and independent skills and individualized support in families where one or both parents have developmental disability. This includes parenting skill development for youth with a disability or delay who have children (single mothers) or families where one or both parents have a developmental disability or delay.
   
 
 

Individuals may choose to receive residential support through living with a family or a room-mate. CORE contracts with the support home (or Community Proprietor) to assist the individual to live in and access their community. Community Proprietors may be contracted to provide full-time or short-term respite support. Respite is provided on a scheduled or emergency basis.

CORE believes that a good match between the individual and the Community Proprietor is the most important factor in developing a stable and successful support home for individuals. Community Proprietors are expected to follow CORE policy and procedure in supporting individuals. They are also expected to encourage and model an accepting and family atmosphere that allows individuals to experience personal growth, and allows them to feel that they are in their own home.

CORE also provides the following services:

  • Behavior intervention programs & support
  • Liaison with schools, medical professionals, social services, department of corrections, community resources based upon individual needs
  • 24 hour on-call
   
 
 

SUPPORTED LIVING: CORE Association has seven residences that provide 24 hour support throughout Medicine Hat Community. Some of the properties are owned by CORE and leased by individuals, others are leased by individuals from community property owners. The level of support offered in the home is determined by the needs of each individual. Although CORE employees provide support, it is home for the individuals, and all individuals are encouraged to participate in the decision making concerning the day to day activities in their home and daily life.

COMMUNITY SUPPORTS (Individualized Funding): Community supports provide one-to-one primary support including family, employment, vocational, independence living and community access support. Services are available to individuals from birth to seniors. We provide support to Medicine Hat and surrounding areas.
